Please use the following instructions to complete the form.

•  Complete sections 1-20
•  Do not complete anything past 20
•  Bring the application in and sign in front of the Clerk of Court or Deputy.
•  This application must be accompanied by 2 (two) passport photos and a certified copy of your birth certificate.
